The role you’ll contribute:

The Care Coordinator serves as an advocate and resource for the patient, their family/caregiver and their health care team to coordinate care needs and community resources to improve health outcomes and decrease the overall cost of patient care. Facilitates a "shared goal model" within and across settings to achieve coordinated high-quality care that is patient and caregiver centered. Builds effective relationships in the community and across the medical continuum of care to strengthen care coordination

  • Demonstrates comprehensive knowledge of organizational and community resources available and develops robust local/regional community social service referral sources
  • Addresses and works to resolve patient concerns or barriers to treatment plan adherence.
  • Assesses and identifies strategies to improve the health literacy of the patient and, as appropriate, family and other caregivers.
  • Assists patients and caregivers with quick appointments and access to healthcare needs.
  • Ensures effective collaboration and communication between the patient and members of the healthcare team to support care coordination activities and goal attainment (i.e. appointment details, health reminders, access to medication, decreased inappropriate use of the emergency room for non-emergent care).
  • Serves as a point of contact, advocate, and informational resource for patient, family, care team, payers, and community resources and facilitate and attend meetings between patient, families, care team, payers, and community resources, as needed.
  • Contribute to build out of the employer program and oversee the day-to-day program operations
  • Maintain ongoing quality improvement plan.
  • Coordinate patient satisfaction events:

o Call backs (when, who, how to report results)

o Mailers

  • Promotes and participates in organization and community activities focused on the health of a patient and their community
  • Provides timely, clear, thorough and accurate documentation of all interactions with patients, and other individuals on behalf of patients, in the patient's electronic health record.
  • Performs all other duties as assigned.
  • Maintaining and growing business from local sources/clients (City governance, ISDs, large or small corporations, etc…). Stays within allocated budget, appropriately manages and reports timely in budget spreadsheet.


  • Responds quickly to issues identified by clients, skillfully communicating to the appropriate administrator(s), and responds back to the clients.

  • Implements strategic marketing plans and strategies, understanding profitability, sets appropriate tracking mechanisms. Works with strategically identified clients to facilitate introductions to programs and services with a focus on building the hospital’s business.

  • Persistent in overcoming objections, while identifying and following through with opportunities. Assist with the development, distributes and communication of key information.


  • Maintains and grows accurate knowledge of current trends in the healthcare market and client arena. Tracks key visits, blitzes, opportunities and issues. Completes event reports, including ROI.
Qualifications


The expertise and experiences you’ll need to succeed
:

Minimum qualifications:

  • High School Grad or Equivalent
  • 3-5 years experience in clinical or community health settings in Care Coordination, Case Management or Healthcare experience
