JOB SUMMARY
The Field Coordinator acts as a liaison between consumers, personal care assistants (PCA), and Consumer Direct. Coordinates home visits for intake, reassessments, and semi-annual visits to assure consumer needs are appropriately met.
JOB DUTIES

  • Maintain confidentiality regarding all communication/information related to consumers, employees, and all business relations.
  • Act as a troubleshooter for inquiry calls or drop-in home visits regarding general program information or for consumer complaints.
  • Attend educational meetings, training sessions, and other meetings as needed or required by the program.
  • When conducting intake visits, complete necessary documentation, and verify Medicaid eligibility. Discuss options for Legal Representation if necessary.
  • Conduct all training with consumer/Legal Representative - review new service plan and train on timesheets. Complete all necessary service documentation
  • Review applications for completeness and assist with fingerprinting if necessary.
  • Correspond with the office regarding any timesheet issues and correction of errors.
  • Perform all outreach/education in assigned territory.
  • Develop in-depth knowledge of program requirements.
  • Other duties as assigned

QUALIFICATIONS

  • High School diploma with 2 years’ experience in the human services field preferred.
  • Ability to read and interpret documents
  • Ability to write routine reports and correspondence
  • Ability to solve practical problems
  • Must be people-oriented, pleasant, and gracious in manner
  • Must be able to pass a background check

While performing the duties of this job, the employee is regularly required to use hands to finger, handle, or feel; reach with hands and arms, and talk and hear. The employee frequently is required to stand, walk, and sit. The employee is occasionally required to stoop, kneel, or crouch. The employee must occasionally lift up to 20 lbs. Specific vision abilities
required by this job include close vision and distance vision.
