Job Description
The Health Navigator Supervisor will be responsible for managing the day-to-day activities of the Health Navigation team. This team provides support and resources to patients to navigate the healthcare system, identify and address social determinants of health, and connect patients to community resources. The Health Navigator Supervisor will ensure that all patients receive timely and appropriate care, as well as oversee the development and implementation of patient care plans.
Key Responsibilities:
- Supervise and provide guidance to the Health Navigation team, including recruiting, training, and evaluating team members.
- Develop and implement policies and procedures to ensure efficient and effective operations of the Health Navigation team.
- Collaborate with other departments and providers to identify patient needs and ensure the timely delivery of quality care.
- Monitor team performance and provide feedback to ensure adherence to quality standards and patient satisfaction.
- Analyze data and develop reports to track team performance and identify opportunities for improvement.
- Ensure compliance with all regulatory and accreditation requirements.
- Assist in the development and management of the departmental budget.
- Participate in departmental and organizational initiatives to improve patient care and outcomes.
Qualifications:
- Bachelor's degree in a health-related field, business-related field, or equivalent experience.
- Minimum of 3 years of experience in care coordination or related field, with at least 1 year of supervisory experience.
- Strong understanding of healthcare systems, social determinants of health, and patient-centered care.
- Excellent communication, organizational, and leadership skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Experience with data analysis and reporting.
- Proficiency in Microsoft Office and Electronic Health Records (EHRs).
Working Conditions:
This is a full-time position that requires occasional evening and weekend hours. The Health Navigator Supervisor will work in an office environment with frequent interactions with patients, providers, and other healthcare professionals.
