Liaison Representative- West Coast applicants ONLY (REMOTE)
Are you an excellent communicator, tech-savvy, have a sales background and a fast learner? Looking for a VERY MOTIVATED and EXPERIENCED customer representative to join our team as a Liaison Representative for PrimoCare.

This position will require you to be tech-savvy, and comfortable with patient outreach. If you are applying to be a PrimoCare Liaison you are willing to work as an independent contractor (no benefits offered until after probation) and comfortable with telephonic outreach and engagement. Additionally, you are caring, patient, persistent, understanding, respectful, able to build trusting relationships, flexible, motivated, and a self-starter.
Duties and Responsibilities:
- In this role, you will be part of a growing team working to improve our patients' healthcare experience.
- You will be representing PrimoCare as you develop a relationship of trust with our patients and facility partners.
The role of a liaison representative is to be a:
- Patient advocate to our senior patients who are starting remote patient monitoring programs.
- Help patients set up their devices.
- Enroll patients and serve as the point of contact for patients enrolled into our Remote Patient Monitoring program.
- Engage regularly with all assigned patients to encourage consistency with the treatment plan and ensure they remain within the data range set by their providers.
- Schedule regular monthly check-ins with the patients.
- Attend virtual meetings with the PrimoCare team for information updates/team collaboration.
Minimum Qualifications:
- Must be an excellent communicator.
- MUST HAVE SALES BACKGROUND
- EXPERIENCED WITH DATA ENTRY
- Spanish speaker is a plus.
- Proficient with technology.
- Comfortable with navigating computer platforms and familiar with smartphone functionality.
- Comfortable with telephone outreach and engagement.
- Experienced with customer interaction.
- Excellent communicator (written and verbal).
- Highly organized and efficient.
- Detail-oriented and good at multitasking.
- Team Player.
- Exhibits a strong sense of ambition, self-motivated, and self-disciplined.
Preferred Additional Qualifications:
- Telemarketing or customer service experience.
- People-oriented
- Proactive and a fast learner
Expectations and Compensation:
- 8hour shifts. Hours between 10AM -630 PM PST (Independent contractor - Full time position) No benefits until probation period completed after 3 months of hire.
- Will receive training.
- Will report to the Care Team Manager.
