The Bannasch Rehabilitation Coach is directed by the Registered nurse. She/he works in a self-directed work team committed to a common purpose and goal. The Bannasch Rehabilitation Coach delivers care to the patients under the direction from the rehabilitation team and must be able to perform satisfactorily the specific patient care skills as outlined by the facility and the department. Must effectively interact with patients, families, and other health team members. Must report pertinent information to the team as needed, and respond to requests for information in a timely manner. The Bannasch Rehabilitation Coach contributes to positive outcomes and meets the cultural expectations of the organization by exhibiting professional behavior. She/he shares responsibility for all aspects of patient and team functions.

  • Standard Work Duties: Bannasch Rehab Coach
  • Actively participates in team development, achieving dashboards, and in accomplishing department goals and objectives. Provides exceptional healthcare while maintaining good stewardship by effectively utilizing department resources and maintains a high level of productivity and efficiency to ensure consistent patient care.
  • Under the direction from the Registered nurse, performs direct patient care, with emphasis on goals set by the rehabilitation team, utilizing established standards/competencies, policies and procedures so that optimum patient outcomes are achieved. Patient and staff safety are a priority in all activities.
  • Under the direction of the therapist, assists and encourages patients to: complete self care/ADLs as indepedently as possible, mobilize as independently as possible at each interaction throughout the day and night, follow recommendations for safe oral intake (swallowing strategies, etc.), and utilize established therapuetic strategies to maximize safety and indepedence in the presence of cognitive limitations. Collaborates with team members to ensure patient's well being and promote optimum patient outcomes.
  • Under the direction of the therapist, is able to select appropriate equipment and method of patient transfer and demonstrates safe use of all patient transfer equipment /device use, patient mobility techniques, and body mechanics. Examples of transfers to be performed include; bed to chair, toilet/commode, or W/C and return, bed to stretcher and return and repostitioning up in bed or chair.
  • Demonstrates knowledge of the individualized care plan for each patient; contributes to the care planning process by providing specific information and observation of the patient's needs and preferences. Inspires patients to participate in rehabilitation goals in a high energy, positive manner.
  • Answer all patient call lights promptly; follows up as appropriate with supervisor regarding reported complaints, problems and concerns.
  • Utilizes measures to promote and maintain patient, visitor, and staff safety, including appropriate use of infection control principles and personal protection equipment.
  • Maintains patient, employee, physician,, and organization confidentiality; maintains HIPPA standards; respects the rights, privacy, and property of others.
  • Promotes patient, physician, staff, and guest satisfaction by supporting the mission, values, and vision of LRMC.
  • Communicates effectively with team members, patients, and families using appopriate written and verbal communication skills. This includes ensuring patient care documentation is complete, accurate, and timely (vital signs, pt. readiness for activity, pain level, etc.) and observes patient for changes in condition or behavior and promptly reports these changes to appropriate rehab staff.

Essential:
  • High School or Equivalent
Nonessential:
  • Certificate of Completion

Education equivalent experience:
Nonessential:
  • Completion of approved PCA and Rehab Coach training program



Other information:
Experience Essential:
  • Experience in Customer Service that required a can do attitude. Experience as a coach/motivator.

Certifications Essential:
  • BLS
  • Per department or hospital requirements and relevant policies.
