Job Description

We don’t accept the broken health care system. We break down barriers to care and show up for members in their moments of need. We deliver on our promise, so members rely on us first to get the care they need - allowing them to recover and stay at home.
We want to help members navigate the healthcare system in a better way, and case management is critical to achieving this for our members. You'll be responsible for providing telephonic case management support to our members newly discharged from the hospital for a thirty-day program. You'll serve as an advocate for these members, coordinating care and ensuring they have the necessary resources and support to achieve better health outcomes and preventing readmissions.
Our Transitions of Care (TOC) Team serves as an advocate for our members, coordinating care and ensuring members have the necessary resources and support to achieve their health goals during an especially critical time. As a Supervisor on the TOC Team, you will be managing and supporting clinical coordinators and nurses to be successful in providing excellent care to members, including: training and coaching, oversight of queues, and improvement of workflows and processes. Our Supervisors build a culture that enables their teams to do their best work.
Responsibilities will include:
  • Leading a team of 10-15 LPN’s, RN’s and/or Clinical Coordinators at varying skill levels
  • Managing your team to achieve goals related to call productivity and clinical quality
  • Reviewing member cases for appropriate level of care, assignment, and workload balancing
  • Quality coaching through conducting call audits
  • Conducting 1X1 feedback sessions and team meetings
  • Invite feedback openly and give it in return
  • Growing the talent on your team
  • High responsibility to understand the roles and act as subject matter expert
  • Ensuring processes are followed and understood
  • Speak with escalated members in a professional and courteous manner in an effort to reach an effective resolution
  • Analyzing trends based on data to close performance gaps and identify areas of opportunity
  • Effectively interview and recommend candidates for hire
  • The ability to articulate and break down complex information
  • Adaptability and comfort in a dynamic, startup environment

Desired skills and experience:
  • An unrestricted RN license
  • A minimum of 3 years of leadership experience
  • Proven success in building relationships and engaged teams
  • A minimum of 3 - 5 years RN experience
  • A minimum of 3 -5 years experience in Managed Care / Health Plan
  • Proficient with technology, Clinical Documentation Systems
  • Understanding of CMS guidelines and MA requirements
  • Bilingual Spanish a plus
Salary Range: $97,689 - $104,000 annually
