A great facility on the south coastal part of Oregon is seeking a full time, permanent Home Health Physical Therapist to join their growing team. Relocation reimbursement available! Details are below:

Pay: $40-$45/hourly + Relocation assistance

Home Health Physical Therapist Opportunity:

• Monday-Friday
• Overtime Options available
• Setting: Home Health
• Will be needing to travel max about 70 miles to see patients
• Home Health Physical Therapist will see about 4-6 patients a day

Home Health Physical Therapist Job Description:

• Provide physical therapy treatment and patient care according to hospital, state and federal regulations and professional guidelines
• For Home Health, develops Physical Therapy Plan of Care including treatments, procedures, modalities, problems, and goals
• A visit to the patient's residence at least once a month, either when the assistant is present to observe and assist or when the assistant is absent, to assess relationships and determine that the plan of cares is being followed, and the goals are being met. Documentation of supervisory visits will be maintained in the patient's clinical record

Home Health Physical Therapist Qualifications:

• Current PT Licensure in good standing
• BLS cert obtained before start date
• 2 years of home health experience required
• Experience with the electronic record program- Well-Sky preferred
