Job Summary
Represents Health Information Management and RUMC internal and external to the medical center, accurately following all applicable federal, state and local guidelines for the processing of patient information. Principally responsible for retrieving patient records/documents for user and maintaining the record in the order designated by the department and hospital. Maintains a demeanor and appearance appropriate for representation of RUMC. Exemplifies the Rush mission, vision and values and acts in accordance with Rush policies and procedures.

Job Responsibilities:
Processes requests for all records from all requestors with courtesy, patience, accuracy, and completeness. Ensures requestor is kept current on the status of the records requested, pulled, and/or copied.
Validates adequacy of written and telephone requests for copies or other information from the patient medical records.
Perform duties related to birth/death certificates.
Scans and performs quality review of images.
Maintains scanning equipment.
Utilizes several software applications to locate and reproduce electronic, paper, film or scanned medical records.
Manages all department off-shift inquiries independently.
Responds to pages from patient care areas for copying, and copies pertinent information from the patient medical records.
Enters abstracted data into system for reporting purposes.
Maintains productivity logs, reporting data to immediate supervisor as requested.
Collects patient charts throughout the medical center and ensures all records are retrieved daily.
Makes rounds' following a specific route and schedule to retrieve all inpatient, outpatient and ambulatory clinic patient records.
In emergency situations or as requested, releases patient information to other hospital emergency rooms or physicians in accordance with department policies and procedures.
Places single and multi-volume patient charts in universal order according to RUMC guidelines.
Print electronic transfer medical records.
Picks up and delivers mail.
Activate MyChart account requests.
Creates and affixes worksheets, barcode labels and color-coded number tags to charts according to RUMC guidelines.
Conducts follow-up with Release of Information to locate charts.
Issues medical record numbers according to RUMC guidelines.
Maintains patient chart location status in software programs with accuracy and completeness.
Maintains all equipment in proper working order, reporting all problems as soon as they are noticed.
Prudently uses supplies reporting shortages as identified.
Assists with copying, filing, mail backs, and other tasks requested.
Assist physicians courteously by pulling records and reports required for their attention.
Sort and deliver signed reports to Physician mail boxes and as directed.
Instruct physicians on use of dictation system.
Sort, identify, and file transcribed reports in proper location of record, regardless of record location.
Move documents from transcription application to EPIC.
Revises transcribed documents occasionally as instructed by manager.
Monitor reports requiring signature.
Update deficiency system for current status of physician documentation deficiencies.
Prepare invoices and other reports on record request status.
Quality Management Responsibilities: 98% prepping and scanning accuracy achieved within 3 months, 100% if paper documentation scanned within 24 hours of receipt, and 5 day turn-around for release of information.

Required Job Qualifications:
  • High School Graduate.
  • Medical terminology (must complete with a C or above grade within first 12 months of employment).
  • Windows application experience, necessary.
  • Interacts well with others.
  • Advanced PC, math, spelling, and alphabetizing skills.
  • Demonstrates attention to detail and accuracy.
  • Proficient in computer use and software applications such as Word, Excel, Access, etc.
  • Telephone, copier, and fax machine skills.
  • Minimum typing skills of 15-20 wpm.
  • Legible penmanship.
  • Ability to concentrate on task at hand in open distracting environment.

Preferred Job Qualifications:
  • Some college courses in accounting, healthcare or secretarial courses.
  • Prior office, healthcare, or Health Information Management experience.

Physical Demands:
  • Ability to move about areas of the medical center throughout the shift.
  • Ability to transport charts, paper, records and items throughout the medical center.
  • Ability to lift up to 50 lbs.
