Admissions Screener duties include:

• Directly responsible for being the liaison between the SNF and hospital/community agency,
• Completing the pre-admission screen assessment,
• Conducting patient/ family interviews and
• Collecting pertinent medical and financial data for Admissions to review.
• Expected to travel from one provider to another and to utilize hospital EMR.
• Must be knowledgeable of admissions practices and procedures, as well as related laws and regulations and guidelines pertaining to nursing home/hospital operation and Insurance benefits.
