On an average day as a Social Worker, you will:

  • You will have the opportunity to collaborate with The Director of Clinical Services and other clinical team members (physicians, chaplains, therapists) to contribute to that mission. These passionate team members will depend on you to provide social services to patients and their families and to be a reliable go-to problem solver.
  • Offer short-term individual counseling, crisis intervention
  • Assist in providing information and preparation of advance directives
  • Assist with funeral planning issues and transfer of responsibilities regarding fiscal, legal and health care decisions
  • Identify support systems available to reduce stress and facilitate coping with end-of-life care
  • Identify and utilize appropriate community resources and assessing patient/family ability to access them
  • And perform additional, related duties as needed

Desired Skills/Qualifications

  • Passion for providing the best possible care to patients and their families

  • Have a valid drivers license, auto insurance, and reliable transportation for the purpose of traveling to our patients
  • At least one year of hospice experience
  • A Masters degree in Social work from a school accredited by the Council on Social Work Education
  • A valid license within the state you are applying
