Summary:
Care Initiatives Hospice is seeking an Admission Coordinator to join our team! The purpose of this position is to coordinate the referral-to-admission process for hospice. The Admission Coordinator will meet with patients and/or families to discuss hospice services and eligibility requirements.

Key Responsibilities:
  • Interview potential patients and patients’ representatives to collect accurate data.
  • Meets with referral sources to discuss appropriateness of referrals and eligibility of patients.
  • Complete pre-admission and admission inquiry forms accurately.
  • Verifies medical insurance and collects medical records.
  • Acquires signed patient consents to elect hospice and completed admission packets.
  • Schedules admissions according to the Admission Staffing Schedule.
  • Communicates status with patients/family, business development, and operations.
  • Coordinates referral-to-admission process, focusing on the next item needed to proceed with admission.
  • Drives to referral sources and patient/family homes throughout service area.
  • Participates in daily growth standup calls to discuss referral status.
  • Conducts post-admission calls to understand patient/family experience.
  • Acts as a liaison between patient, family, business development, and operations.
  • Performs other related duties as directed by his/her supervisor.
