Location: Houston, TX

Pay: $16,502 per year stipend (required # of hours: 1,700/year)

HERE'S WHAT YOU'LL BE DOING:

The community health worker will serve as a liaison/connector between their client, the heath care team, and the community. Helps to bridge conversations with clients and remove barriers that prevent them from accessing health and social services.

Day-To-Day

• Develops trusting relationships with clients by providing support and advocacy to help achieve health goals
• Conveys the purpose and services of Community Health to the member population and the impact that program or service.
• Meets member in clinic, facility, office, or at home to help identify social determinants of health impacting member's health and general well-being
• Follow-up with health management/care plans with both members and providers
• Coach member in effective management of their chronic health conditions and self-care.

Qualifications

• High School diploma or equivalent

• Any combination of 1 years health/social services experience and/or education

• Prefer candidates who live within a 10 mile radius of zip codes 75208 & 75211
• Valid Texas Driver s License and ability to be insured.
