We’re currently looking for an exceptional LPN Nurse Care Manager just like YOU to help us change the way healthcare is being delivered across the WORLD.
    The ideal candidate for this role is compassionate, flexible, self-motivated, detail oriented, and a constant learner. Under the supervision of the Chronic Care Management Supervisor, you will provide longitudinal care management services to patients enrolled in our Chronic Care Management Program. This is an onsite telephonic position located at our headquarters in Southfield MI. Candidate must reside in the Metro Detroit area*
    When you become our next LPN Nurse Care Manager you’ll take on:
  • Ensuring the delivery of high-quality care by assessing and addressing patients’ individual health needs
  • Providing self-management education to ensure that patients have the tools they need to manage chronic conditions at home
  • Working collaboratively with patients and PCP to create individualized care plans that include SMART goals
  • Managing assigned patient case load to ensure that performance goals are met each month
  • Using motivational interviewing techniques and active listening to build a rapport with patients during telephonic visits
  • Documenting each patient encounter and tracking progress in the patients EMR and other software systems
  • Helping patients to navigate the healthcare system by providing assistance with care coordination and referral management
  • Connecting patients to appropriate resources within and outside the Epic Health System
  • Identifying patient health trends and providing clinical coaching to help improve patient outcomes
  • Identifying gaps in care and proactively addressing, educating, and closing gaps during patient encounters
  • Working collaboratively with members of the interdisciplinary team to ensure quality care
  • Triaging patient concerns and directing them to the most appropriate point of care
  • Screening patients for barriers to wellness and following up with appropriate resources
  • Reviewing current medications with patients and completing medication reconciliation to improve compliance
  • Building patient/caregiver rapport by providing compassionate and dependable care
  • Maintaining patient confidentiality and compliance with HIPPA
  • Participating in regular process improvement projects and experiments to help improve the office flow and patient care
  • Training other staff members as needed

  • You would be a great fit for this role if you have:
  • Current and unencumbered LPN license in the state of Michigan
  • 1-3 years case management, disease management, or behavioral health care experience
  • At least 3 years of verifiable clinical experience working with adults who have chronic conditions
  • Telephonic or telemedicine experience preferred but not required
  • Knowledge of current healthcare practices and acceptable medical treatment
