This is a part-time hourly position with the Pikes Peak Area Council of Governments, Area Agency on Aging. The position is a Care Planner for informal caregivers who contact the Center for assistance. As the part-time Care Planner this individual is primarily responsible for providing information and assistance to informal caregivers to help them access needed services in the community. This position assists informal caregivers to manage their caregiving responsibilities by developing a plan to support them in caring for an elderly spouse, parent, family member, or friend.

Duties include but are not limited to:

• Develop individual care plans for caregivers who contact the Center for information and services
• Return phone calls from caregivers who call the Family Caregiver Center seeking support and assistance
• Assist caregivers who walk into the Area Agency on Aging office seeking support and a consultation
• Refer individual caregivers to the proper services and assist them in accessing those services
• Assist the Family Caregiver Center Coordinator with issuing monthly caregiver respite vouchers
• Participate in community senior events involving the Family Caregiver Center, as needed
• Serve as a back-up for the Family Caregiver Center Supervisor when the Supervisor is out of the office for an extended period of time
• Perform other work as requested by the Family Caregiver Center Supervisor

Required Education and Experience

• Bachelor’s degree in a human services curriculum or related field plus a minimum of two years of progressively responsible experience with an agency serving human resource needs, or
• May have commensurate work experience of at least four years in lieu of a degree
• Basic computer skills including Microsoft Word and Excel and internet usage. Database experience is helpful.

Desirable:

• Knowledge of community resources for the elderly and for caregivers of the elderly, as well as of community service agencies;
• Good speaking and written communication skills and good listening skills
• Basic computer skills including Microsoft Word, Excel and knowledge of using the Internet
• Bilingual

A background check and Colorado Adult Protective Services (CAPS) report will be run

Pay: $25.00 - $26.50 per hour

Experience:
