Position Summary: The Population Health Manager helps clients improve health care quality and achieve ACO Shared Savings. The Population Health Manager achieves these aims by carefully tracking performance metrics, serving as a project manager, and training clients to effectively implement Signify Health Foundational Programs such as team-based care, annual wellness visits and hierarchical category coding. The Population Health Manager facilitates monthly calls with primary care practices, builds strong client relationships and identifies opportunities where additional subject matter expertise should be deployed.
