Position Overview:

To perform medical records functions such as scanning medical information into the EHR system, printing electronic records and/or retrieving charts from storage for patients, legal or medical requests, subpoenas, while complying with HIPAA and other regulatory requirements. Maintain a high degree of performance and efficiency, resulting in 100% availability of all records. This work is carried out in support of the mission and goals of Ryan Health.


Essential Functions:

  • Responsible for scanning lab reports, referrals and other pertinent reports into the EHR, in an accurate and timely manner.
  • Perform electronic document validation, ensuring that all electronic documents scanned are organized and filed in the appropriate patient’s electronic health record.
  • Locate, retrieve and deliver medical records, both paper and electronic, for various reviews, studies and audits, Center policy and procedure in accordance with and in compliance with HIPAA.
  • Responsible for the printing of medical information from the EHR system or the retrieval of all medical record for audits, patient requests and subpoenas.
  • Perform computer merge function to merge duplicated patient entries in EHR.
  • Release information to persons or agencies in accordance with Ryan Health’s policies and procedures and incompliance with HIPAA and other regulatory requirements. Maintain logs as required by HIPAA. Answer phones and respond to telephone requests from staff or patients regarding medical records and/or medical information.
  • Assist patients who present to medical records to have forms completed or to request a medical letter. Assure that proper patient authorization is obtained; verify on the EHR system that patient had necessary physical exam and tests necessary for the completion of the form/letter; schedule patient pick-up dates; complete a draft of the form or letter; take form/letter to practitioners for their completion, review and signature; scan completed forms/letters into the EHR system, and maintain alphabetical file of completed forms/letters to be picked up.
  • Must be familiar with all Medical Record procedures and cover areas of work as needed.
  • Attend department meetings and other meetings as required. Participate as a part of a team in performance improvement activates.
  • Immediately report any problems and/or unusual occurrences to supervisor.
  • Perform other duties as assigned.


Minimum Experience and Skills Required:

  • One year of medical records experience, including correspondence.
  • Experience with Electronic Health Records.
  • Data Entry and in/output control experience.
  • Ability to maintain patient confidentiality at all times
  • Must enjoy working with the general public


Education, Licenses and/or Certifications Required:

  • High School Diploma or Equivalency.


Preferred Qualifications:

  • Experience in legal aspects of medical records, i.e. lawyers’ requests for medical records; subpoenas.
