Pay Rate: 21-30 DOE

Job Summary: Responsible for providing Health Information Management support to providers, staff, and patients. Responsible for analyzing and monitoring discharged patient records to ensure timely completion of medical record documentation. Responsible for monitoring account work queues to support timely Coding and Billing. Works closely with the Medical Staff office to ensure accuracy with provider administration. Performs quality control and audits for the electronic health record (EHR). Produces hospital and HIM statistics. Supports transcription access, training and processing.

Job Responsibilities: (**Essential job function)

1. **Analyze Inpatient, Outpatient, and Clinical discharged charts.

  • Technical review of medical record documentation to ensure compliance with State and Federal requirements, accreditation standards, and Medical Staff Rules and Regulations.
  • Enter deficiencies into the EHR system.

2. **Monitor documentation deficiencies.

  • Generate weekly notification letters to providers, assess suspension status, and determine fines status.
  • Build strong, collaborative relationships with providers and staff to ensure timely documentation.
  • Send daily Scheduling Hold updates to internal customers as well as those providers with deficient operative reports.
  • Closely monitor deficiency and account workqueues and reports to support timely Coding and Billing.

3. ** Participate with ensuring accuracy with Epic Provider Administration.

  • Respond to Medical Staff office notifications and requests.
  • Run reports to ensure suspended providers are in an appropriate status

4. **Provide support for Release of Information (ROI), including after-hours ROI for continuity of care. Process messages in the Epic Patient CRM Request inbasket.

5. ** Participate in all system training and meetings to stay up-to-date on all changes and versions to system.

6. ** Ability to review, correct errors and ensur all assigned work is complete and accurate.

7. **Provide support of scanning process, transcription support processes, and validation of scanned images into EHR. Obtain dictation access from vendor, coordinate interactions with vendor, and train providers regarding dictation. Assure electronic upload of reports via interface processes.

8. **Monitor CareEverywhere and health information exchange (HIE) to support Opt Out choice for patients.

9. Comply with HIPAA guidelines and Security rules at all times.

10. Follow the policies, procedures, and practices outlined in the Code of Conduct and Organizational Ethics policy. Report any business practice/compliance concerns according to organization policy.

11. Exhibit behaviors that support the organization's Service Standards and Cultural Beliefs.

12. Other duties as assigned, including assisting with Medical Record Technician I duties when needed.

  • Maintain the ability to perform all job duties identified in the Job Summary
  • Review and evaluate the workflow of the Health Information Management Department
  • Prioritize and direct the activities of other employees within the department, assisting with coverage and task assignment
  • Report any observed behaviors of employees, whether favorable or unfavorable, and provide observed performance data to Manager
  • Assist by monitoring performance related to a Performance Improvement Plan, initiated by the Manager.
  • Assist co-workers in times of increased workload or temporary reduced staffing
  • Serve as a resource person or problem solver for those employees performing similar functions within the department.

Minimum Qualifications:

· Education: High school or equivalent required: Associates of Arts degree in Health Information preferred. Medical terminology coursework required.

· Professional Licensure/Certification/Registration: Registered Health Information Technician (RHIT) or eligible to sit for t'he exam preferred.

· Experience: Six months prior medical record experience required.

Special Training/Skills: Critical thinking skills are a requirement of this position. Computer proficiency is required including knowledge of Word and Excel. Strong verbal and written communication and customer service skills including the ability to communicate effectively across a wide range of audiences. Ability to work both independently and as part of a team.

Working Conditions: Well-lighted, climate-controlled office environment. Minimal potential for exposure to fumes, noise, chemicals, adverse weather conditions.
