Job Title: Outreach Coordinator

Reports To: Director of Clinical Services

Type: Full-Time Temporary Grant Funded Position (through March 2024)

Location: In the Community

I. Purpose

The Outreach Coordinator, under the supervision of the Director of Clinical Services, is responsible for building relationships and partnerships for the organization at the local level with special emphasis on referral development partnerships with other healthcare partners, social and human service agencies, community centers, healthcare providers, and more. Conduct outreach, facilitation and education to potential clients, former clients, people with substance use disorders and/or their family members, organizations and professionals in the community. The Haven endorses a trauma-informed care approach and a medical model of SUD treatment.

II. Essential Functions

 Cultivates and steward mutually beneficial, long-term relationships with gatekeepers from a wide variety of community sectors (e.g., business, healthcare, education, child welfare, funders, referral sources and others) at the local level and leverage them to convert referrals into enrolled clients.

 Identifies opportunities that enhance the resources, reputation and relationships of the organization.

 Educates referral sources on the value of The Haven as the provider of choice in the marketplace and continuously research the market to identify new, appropriate referral sources with whom to partner.

 Establishes and maintain effective working relationships with The Haven’s program staff, volunteers and interns.

 Monitors phone calls, voice mail messages, email messages and social media to develop and maintain a list of persons and organizations in need of education and/or resources for treatment of substance use disorders.

 Engages in internet research and interviews with people in the southern Arizona community to develop and maintain a list of persons and organizations in need of education and/or resources for treatment of substance use disorders.

 Makes regular contact (at least weekly or more often as indicated) with persons and organizations in need of education and/or resources for treatment of substance use disorders until their needs are fulfilled.

 Facilitates and/or provide education regarding substance use disorders and treatment with persons and organizations in need of education and/or resources for treatment of substance use disorders until their education needs are fulfilled.

 Coordinates substance disorder treatment admissions (with The Haven as well as other providers) for persons in need of such treatment.

 Completes screenings for women seeking admission to The Haven for treatment of a substance use disorder.

 Provides supportive, educational and social activities to former clients of The Haven.

III. Other Duties and Responsibilities

 Maintains records of all outreach, facilitation, education and coordination activities including persons and organizations involved.

 Works with leadership to aggregate data as indicated.

 Enters data regarding pre-screenings into the electronic health record system and maintains all protected health information in accordance with federal laws and rules.

 Completes all tasks in a timely manner based on time frames established by the Director of Clinical Services.

 Establishes and maintains a professional working relationship with all staff and professionals in the community.

 Provides excellent customer service including the maintenance of professional boundaries with persons and organizations in need of education and/or resources for treatment of substance use disorders.

 Maintains adequate records of all miles driven, along with other travel costs, in order to receive reimbursement.

 Performs other duties as assigned by the Director of Clinical Services.

IV. Qualifications

 A high school diploma or GED is required

 An Associate’s degree is preferred

 Must be able to work evenings and weekends hours if required

 Must possess good customer service skills

 Demonstrates ability to achieve short- and long-term goals

 Demonstrates ability to work independently and assume responsibility for design and implementation of outreach strategies.

 Demonstrates commitment to accountability, outcomes measurement and a results-oriented culture

 Must possess competent computer skills, including use of Microsoft Office

 Ability to communicate the organization’s vision and impact to a diverse group of constituents.

 Ability to present ideas effectively, in both oral and written form

 Ability to prepare detailed reports and prepare recommendations when necessary

 Ability to contribute to a collaborative and professional team.

 Must possess a fully insured and well maintained motor vehicle along with an active license to operate such a vehicle.

 Ability to exercise judgment and maintain confidentiality.

 Ability to build relationships and collaborate in a complex environment with clients, staff, referral agencies, community partners, and others.

 Demonstrates cultural competency by communicating and working effectively with those from diverse backgrounds and creating and sustaining an environment in which people from diverse backgrounds and perspectives can succeed.

V. Extent of Public Contact

Interacts with external entities, with all levels of Haven personnel and with potential and former Haven clients
