Essential Functions:
- Provides twice monthly home visits to enrolled Bright Beginnings families with children from birth to 5 years.
- Develops and maintains positive, supportive relationship with parent and child(ren).
- Teaches effective parenting and child development skills, following the Parents as Teachers curriculum.
- Assists families with making formal and informal connections to appropriate community services throughout the program.
- Provides parents with information on disciplining, behavior management, household management and communication and relationship enhancement when needed.
- Provides crisis intervention and mediation as necessary.
- Available by cellular phone for families in crisis.
- Transports clients as needed
- Provides the direct service to families as required by the program.
- Assists families with developing and attaining goals on a regular basis.
- Completes and updates various assessment tools and screenings within the required timeframe, including the ASQ (Ages & Stages Questionnaire) with the family.
- Assists with planning and hosting once monthly Family Groups.
- Regular and reliable attendance is required for all aspects of the job.
- Coordinates, makes referrals, or arranges for appropriate community resources when needed, and act as an advocate if necessary.
- Teaches families how to utilize the system.
- Ensures that children are in the proper educational setting.
- Maintains ongoing contacts (phone, meetings, conferences) with other professionals involved, including collateral contacts with community resources/services.
- Organizes and maintains client information and case notes.
- Completes required record keeping and copying of necessary information, including dissemination to appropriate individuals within the agency and any outside agencies based on departmental requirements.
- Completes all data entry into Visit Tracker System, accurately and within required timeframes.
- Attends and participates in all Help Me Grow/Bright Beginnings staff meetings and meets with supervisor as determined necessary.
- Ensures that case records are complete, current and accurately maintained.
- Attends and participates in all required internal and external staff training programs.
- Maintains regular and reliable attendance.
- May be involved in the planning, organization, and implementation of special events or department specific projects or seasonal program changes.
- In times of need, employee may be asked to complete related duties other than those indicated above as assigned by their supervisor.
Performance/Physical Requirements:
- Work is performed in the homes of clients. This involves extensive driving. A valid Driver’s License, safe driving record, reliable vehicle and proof of insurance are required.
- English reading and writing skills required
- Interacts verbally with coworkers, representatives from outside agencies, and families
- Attention to detail and confidentiality required
- Works flexible hours
- Requires typing and computer competency, including Windows applications for moderate levels of typing case notes and other paper work.
Qualifications:
Minimum educational level is an Associate’s Degree in a major field of study related to early childhood or be licensed as a registered nurse. Bachelor’s degree in social sciences with related experience is preferred. Minimum age of 21 years, with a valid Ohio driver’s license and proof of insurance. Must obtain and maintain Parents As Teachers 0-3 and Parents As Teachers 3-5.
