Population Health Specialist

Neighborhood Health Association (NHA) is seeking a motivated and detail-oriented individual for the Population Health Specialist position. This position is responsible for ensuring data is accurate, disseminating reports throughout the organization, and training and management of population health tools. This is a great opportunity for someone with an interest in quality improvement looking for an introduction to the primary health care space.


The Population Health Specialist assists in organization-wide efforts to ensure that quality improvement (QI) measures are complete. This position is directly responsible for departmental data management and reporting.

This individual is self-directed and requires minimal supervision. They will collaborate with the Director of Quality to prepare for regulatory surveys and audits.


Duties and Responsibilities:

  • Responsible for the administration and training of the Azara DRVS tool
  • Coordinates with clinical employees regarding data reporting, including data pulls from queries specific to UDS, HEDIS, PCMH, CPC, TJC and FTCA
  • Coordinates with internal and external points of contact for resolution of data or system errors or problems
  • Assists in the coordination of the Quality Improvement Committees, Safety & Risk Committees and assists in convening regular meetings
  • Develops reports to assist individuals and committees in QI processes
  • Ensures required data is collected on a timely basis, regular reports on progress are distributed, and recommendations for future improvements based on the data are accurate.
  • Presents QI department information at employee orientations, in absence of QI Director
  • Performs ad-hoc reports as needed
  • Assists in ensuring implementation and effectiveness of operations within local, state and federal regulatory compliances
  • Works with the EHR trainer to ensure data integrity
  • Works with various data reporting and population health tools
  • Filters, cleans data and validates reports by reviewing data and performance indicators to locate and correct reporting problems
  • Extracts and analyzes data to support the organization’s decision-making processes
  • Provides recommendations to the QI Director based on professional expertise, data analytics, and insights regarding the improvement of care and organizational systems
  • Assists QI Department with medical meetings, planning sessions, and visualization tools
  • Assists with the coordination of training material
  • Assist the Medical Records Manager with tasks including but not limited to duplicate charts and deceased patient lists
  • Other duties or special projects as assigned

Skills and Qualifications

  • Associate’s degree required with a Bachelor’s degree preferred (in a Health or Business Administration related field strongly preferred).
  • Experience in an operations role in a healthcare setting preferred.
  • 2 years quality, clinical or medical record experience preferred.
  • Minimum of 1-year experience using an electronic health record system.
  • Must be highly proficient in Microsoft Suite and report writing tools.
  • Knowledge of QI and/or Total Quality Management Theory and practices preferred.
  • Must be able to work independently and with little or no supervision.
  • Strong ethics and a high level of personal and professional integrity. Must adhere to departmental standards to HIPAA and other privacy concerns.
  • Must have the ability to perform detailed work with a high degree of accuracy.
  • Ability to organize and prioritize workload in order to meet established schedules, timelines, or deadlines with the ability to work independently with minimal supervision to complete assigned tasks.
  • Must be sensitive to cultural, religious, and ethnic diversity.
  • Possesses a neat and professional appearance.
  • A valid Ohio driver's license and auto insurance with an acceptable driving record; reliable transportation and willingness to travel to all NHA clinic and office locations when required.

Full-time, exempt salary position. Monday through Friday no weekends or holidays.
