Population Health Specialist
Neighborhood Health Association (NHA) is seeking a motivated and detail-oriented individual for the Population Health Specialist position. This position is responsible for ensuring data is accurate, disseminating reports throughout the organization, and training and management of population health tools. This is a great opportunity for someone with an interest in quality improvement looking for an introduction to the primary health care space.
The Population Health Specialist assists in organization-wide efforts to ensure that quality improvement (QI) measures are complete. This position is directly responsible for departmental data management and reporting.
This individual is self-directed and requires minimal supervision. They will collaborate with the Director of Quality to prepare for regulatory surveys and audits.

Full-time, exempt salary position. Monday through Friday no weekends or holidays.
