Reliant Medical Group, Part of Optum , is seeking an Advanced Practice Clinician to support the Office of Population Health as a Primary Care Community APC.

This newly designed role will help support Reliant patients that are 18 months or more behind in their care. This role is designed to meet with patients in their home, to support them to re-engage in their care and to help assess and monitor their current health status with screenings, discussions and possibly prescribing.

Ideal candidates will have a strong clinical background and the ability to work autonomously. Home care experience is ideal but not necessary.

Required Qualifications:

• Graduate of an Accredited Nurse Practitioner or Physician Associate Program
• Unlimited Licensure in the state of Massachusetts
• Active DEA License
