Job Responsibilities to include: Coordinating applicant waitlist / communicating with applicants and their families, Coordinating admissions and discharges. Other responsibilities could include participating in ISP (Individual Support Plan) development, Identifying risk and protective factors for residents, Assisting support staff in understanding the family dynamics of the resident's life, Providing education and training to support staff, Assisting families in accessing community resources and providing appropriate referrals to family and residents.
