The Appointment & Discharge Coordinator (ADC) is tasked with providing coordination/transportation and scheduling of medical appointments and discharge planning for Residential Program students.
- Works collaboratively with the Residential nursing department, direct care staff and community providers the ADC takes lead in establishing and facilitating all medical and related appointments.
- Works collaboratively with students and families in the development of discharge plans for Residential Program students post placement under the direction of Residential Clinical Therapists.
- Ensures fluid and dynamic utilization of agency and community resources to assure students are having their medical needs and discharge planning provided.
- Establishes, confirms and as needed transports the medical necessary appointments, consultations, evaluations and treatments, follow up or reevaluation for students while in Residential care.
- Works within the electronic health record and email systems to assure documentation regarding appointments and follow ups are clearly communicated and data is accurately collected.
- Collaborates and proactively creates plans for student discharges including but not limited to attending plan of care meetings prior to discharge, organizing and submitting post-discharge referral services, providing access for discharge placements and clearly enrolling student/family in outpatient services.
- Communicates with appropriate team members regarding the needs of the appointments and discharge plans.

Pay:
- $18.34 + DOE
- 3% Pay increase after successful completion of a 6-month Intro Period
