We have the following units: Rehab, Long Term Care, 4 Specialized Secured Behavioral Health Units including High Acuity, Dementia and SMI specified units.

Ridgecrest Post cute is a 5 Star CMS Rated Facility looking for a 5 Star MDS Nurse Assistant that is detailed, organized, and values working with a high performing team.

This position ensures timely and accurate MDS and care plan process completion according to current state and federal regulations. This positions reports to the Lead MDS Coordinator.

Major Tasks, Duties, and Responsibilities:

• Interviews residents, family and Health Care Team
• Reviews medical record
• Observes/assesses residents
• Completes and transmits MDS according to regulation time frames
• Schedules MDS
• Completes care plans
• Coordinates with HCC team to facilitate completion of MDS
• Participates in care plan process, updates and teaches team members process and changes
• Participates in care plan meetings with resident and families
• Demonstrates knowledge of Medicare reimbursement, PDPM rates and PPS systems
• Communicates Validation report to Administrator and DON
• Knowledge of ICD codes
• Other duties as assigned

Requirements:

Education and Experience:

• Previous MDS experience
• At least 1 years SNF/LTC experience
• Current Arizona RN/LPN Nursing License in good standing
