We are seeking a Patient Navigator to be an integral member of the healthcare team and provide indirect care to patients at our Medicine Specialties of South Durham.

JOB DUTIES

  • First point of contact for people interested Registering and scheduling new patients. Educating them on questions regarding first appt, sending email with appt date/time/location and questionnaires and information regarding first visit.
  • Manage intake surveys ensuring completion and entering into Epic
  • Coordination of services with other specialties (ex: dermatology, surgery, ENT)
  • Obtain and scan in records for new patients for first appt.
  • Scheduling return appts for gender clinic if not done at check out or cancel/reschedule when patients call in.
  • Managing patient waitlist
  • Triaging emails/phone calls from new and return patients and pharmacies regarding questions about meds.
  • Manage and coordinate lab orders and results when labs are done outside of Duke
  • Provide follow up resources for patients on surgical requirements, community resources, mental health services, surgeon-specific requested services, general and surgical financial assistance services.

REQUIRED QUALIFICATIONS

  • Bachelor's degree in a clinical field, such as behavioral/social sciences, health education, or public health fields.
  • Three years of experience in a patient-centered, health related field with administrative duties.
  • Experience in patient navigation preferred.

KNOWLEDGE, SKILLS AND ABILITIES

  • Excellent written, oral and conversational communication skills to effectively work with diverse groups.
  • Ability to analyze, evaluate multiple solutions and solve complex problems using well developed critical & analytical thinking skills.
  • Excellent time management skills needed to prioritize among many competing priorities.
  • Attention to detail needed to operate accurately and effectively.
  • Mastery of Microsoft Office, especially Word and Excel, and electronic medical record documentation and/or reporting.
  • Metric tracking skills
