Mobile Respite Case Manager


The Mobile Respite Case Manager is a member of an interdisciplinary team working together to provide respite and stabilization services to DMH clients. Responsibilities include providing direct clinical services to individuals in the Mobile Respite Programs (individual and group treatment, family meetings, treatment team meetings); clinical documentation (intakes, treatment plans, discharge summaries); case management, and ensuring coordination of care internally and with other treatment providers and community support systems for persons served. The Mobile Respite Case Manager utilizes the Recovery Model and works closely with an interdisciplinary team including Counselors and Peer Specialists to support persons served in self-advocacy and self-directed treatment planning. The Mobile Respite Case Manager works closely with the Department of Mental Health regarding the referral and enrollment decisions for persons served.


Essential duties and responsibilities include but are not limited to:

  • Oversee the case management and discharge planning of Mobile Respite persons served.
  • Evaluate persons served on an ongoing basis to assess for safety and continued need for Mobile Respite services.
  • Complete all necessary clinical documentation, including admission notes, intakes, treatment plans, discharge plans and reporting.
  • Facilitate consents, gather clinical information, and open discussion with enrollee’s providers.
  • Meet with Mobile Respite persons served in the community. May be responsible for transporting persons served to appointments/resources in the community.
  • Ensure timely and accurate billing practices.
  • Assist Clinical Team Manager/Director of Stabilization Services in gathering monthly, quarterly, and annual data to MBHP/ DMH as required.

Schedule: Full Time; Sunday through Thursday- day shift


Pay: $48,000/ annually

Required Skills
  • Bachelor’s in Social Work, Psychology, or related field required.
  • Experience doing Community Case Management/Mobile Crisis Intervention work preferred.
  • Clinical experience with adults and knowledge of the adult system of care preferred.
  • Valid driver’s license and regular access to a reliable vehicle required.


Required Experience
  • Excellent communication, interpersonal and organizational skills required.
  • Intermediate computer fluency, including Microsoft Office, required.
